About the Business: Victorian Home and Community Care Pty Ltd (VHCC) is a registered NDIS provider delivering person-centred disability support services across NSW and Victoria.
We specialise in community participation, assistance with daily living, personal care (including high-intensity supports), supported independent living, community nursing, psychosocial recovery coaching, support coordination and allied health services.
With a team of over 300 multicultural and bilingual care workers, we focus on building independence, dignity and community connection for participants.

About the Position: The Community Worker role is central to VHCC's mission of supporting vulnerable people in the Narromine region and surrounds.
You will assess community needs, deliver informal counselling and life-skills education, and actively connect participants to essential services so they can live more independently and participate fully in community life.
This position plays a key part in translating NDIS plans into practical, meaningful outcomes on the ground.
Key Responsibilities
Assess individual and community needs to identify barriers and opportunities for greater independence and participation
Provide informal counselling and emotional support to participants and their families
Deliver life-skills education and capacity-building activities tailored to individual goals
Connect participants with essential support services, mainstream community resources and specialist providers
Facilitate community participation and social inclusion activities
Maintain accurate records and contribute to support planning and review processes
Work collaboratively with support coordinators, allied health professionals and other team members
Uphold NDIS quality and safeguarding standards, privacy and participant dignity at all times
Requirements
Australian citizens and permanent residents are encouraged to apply
Bachelor Degree in the field
Minimum 1 year relevant experience
Strong interpersonal and communication skills, with the ability to build rapport and trust with people from diverse backgrounds
Sound assessment, problem-solving and organisational skills
Ability to work independently in a community-based setting while following organisational policies and NDIS requirements
Empathy, cultural sensitivity and a genuine commitment to person-centred practice
Current drivers licence and willingness to travel within the local area as required
