About You :
Certificate IV in Disability, Community Services, Individual Support or equivalent
Position Summary :
The Support Worker provides person-centred, high-quality support that enables participants to achieve their goals, maximise independence and live meaningful lives within their home and community.
Working in accordance with the NDIS Practice Standards, organisational policies and procedures, and all relevant legislation, the Support Worker promotes participant choice, dignity, inclusion and safety while maintaining the highest standards of professionalism.
Key Requirements :
Experience supporting people with disability, aged care or community services (preferred).
Excellent communication and interpersonal skills.
Ability to work independently and within a team.
Strong organisational and time management skills.
Ability to maintain accurate documentation.
Understanding of person-centred practice.
Experience supporting participants with complex behaviours.
Mental Health experience.
High Intensity Support experience.
Behaviour Support knowledge.
Experience with manual handling equipment.
Knowledge of Positive Behaviour Support practices.
Job Responsibility :
Participant Support
Deliver individualised supports in accordance with participant care plans and goals.
Promote independence, choice, dignity and self-determination.




Build respectful and professional relationships with participants, families and stakeholders.
Encourage community participation, social inclusion and skill development.
Support participants to achieve their NDIS goals.
Personal Care
Provide support where required with:
Personal hygiene and grooming
Showering and dressing
Toileting and continence care
Medication assistance (within training and organisational policy)
Meal preparation and nutrition
Mobility and transfers using appropriate manual handling techniques
Community Access
Assist participants to:
Attend appointments
Participate in community activities
Access education, employment and volunteering opportunities
Complete shopping and errands
Develop confidence and independence in the community
Domestic Assistance
Provide support with:
General household cleaning
Laundry and linen
Meal preparation
Grocery shopping
Maintaining a clean, protected and healthy home environment
Documentation
Complete shift notes before the end of every shift.
Accurately record medication, incidents and participant observations.
Report risks, incidents and concerns immediately.
Maintain confidentiality of participant information.
Ensure all documentation is accurate, factual and completed within required timeframes.
Health, Safety & Compliance
Follow all WHS requirements and safe work practices.
Maintain participant dignity, privacy and rights.
Follow infection prevention and control procedures.
Maintain mandatory compliance requirements.
Report hazards, incidents and safeguarding concerns immediately.
Participate in mandatory training and professional development.
Teamwork
Communicate professionally with participants, families and colleagues.
Attend meetings, supervision and training as required.
Support a positive team culture.
Work collaboratively with internal and external stakeholders.
