What Your Day Might Look Like • Build genuine relationships with clients, getting to know what matters most and supporting them to live healthy, active and connected lives • Use your experience to complete holistic assessments and create personalised goals that promote independence, wellbeing and choice • Coordinate tailored home care plans, services and budgets, connecting clients with the right supports to achieve positive outcomes at home What You Bring • Experience in case management, community or aged care, with a holistic, goal-focused approach that helps clients achieve meaningful outcomes and live well at home • A natural relationship builder with strong communication skills, able to connect with clients, health professionals and community partners to create positive client experiences • Self-motivated and organised, with the confidence to work independently, manage competing priorities and take initiative when it matters most • A genuine enjoyment of being out in the community, travelling to visit clients and build local connections What You Need to Know • Enjoy the flexibility of a Permanent Part-Time role with four days working remotely • Hit the road with confidence,



with access to a fleet vehicle or daily travel allowance, and enjoy travelling within your community, with journeys typically up to ***** minutes • Qualifications in Allied Health, Nursing, Social Services or a related discipline are valued, but we're equally interested in your experience and passion for supporting older Australians • Successful applicants will be required to complete a National Police Check and provide evidence of current work rights in Australia.
