The Salvation Army Australia is seeking a Transition Coach in Morley, WA. This full-time outreach role supports young people aging out of care through one-to-one coaching, mentoring and coordinated services.
You will help them access housing, education and employment while building strong supports and cultural connections. You will collaborate with Child Protection staff and carers, practicing trauma-informed, culturally protected and strengths-based work to empower young people toward independence
