This is a 12 Month Parental Leave Max Term Contract - 3 days per week

The opportunity
Reporting to the Patient Services Business Manager, you'll work within a close-knit multidisciplinary team, coordinating and supporting patients from diagnosis through treatment and beyond. Your day-to-day will include:
- Providing proactive, personalised support to patients and carers navigating their cancer care pathway
- Coordinating appointments, referrals, and communication across clinical teams to reduce gaps and delays
- Identifying and connecting patients with psychosocial, financial, and community support services
- Maintaining accurate case records and contributing to team reviews to improve care coordination outcomes
What You'll Bring
- A background in nursing, allied health, or a related clinical discipline with current AHPRA registration or relevant qualifications
- Experience in oncology, chronic disease management, or complex care coordination settings
- Demonstrated ability to communicate sensitively with patients facing serious illness and their support networks
- Strong organisational skills with the ability to manage competing priorities across a multidisciplinary setting
- Familiarity with electronic medical records and care coordination tools
- An understanding of the psychosocial dimensions of cancer care — something central to the Lifehouse model
Essential Requirements
- Registration with the Australian Health Practitioner Regulation Agency (AHPRA) - if applicable
- Legal right to work in Australia - we regret no sponsorship is offered for this position
- Current Working with Children Check (WWCC)
- National Police Check (within the last 6 months or willingness to obtain)
- Compliance with Category A immunisation requirements
