About the Role
As part of our team, you will play a vital role in delivering coordinated, patient-centred care to veteran patients aged 25-65. The average age of the patient cohort is 36.
Many of our patients are managing chronic health conditions and complex healthcare needs.
Your role will be to guide, support, advocate, educate, and coordinate care that helps them achieve the best possible health outcomes.
Key Responsibilities
- Deliver holistic, compassionate care that addresses physical, emotional, and psychological wellbeing.
- Coordinate and develop comprehensive care plans that support long-term health outcomes.
- Navigate complex healthcare pathways, ensuring patients and families receive appropriate support and services.
- Conduct detailed health assessments and identify opportunities for preventative intervention.
- Coordinate communication across multidisciplinary teams to ensure seamless patient care.
- Support chronic disease management and preventative healthcare programs.
- Advocate for veteran patients, helping them access the care and services they need.
Clinical Skills & Duties
You will have the opportunity to utilise and further develop a broad range of clinical skills including:
- Assisting with minor procedures including excisions and biopsies.
- Iron infusions and cannulation (highly regarded).
- Wound care management.
- Emergency first aid and clinical triage.
- Immunisations and preventative health initiatives.
- Health screening clinics including diabetes and asthma management.
- Patient education
- Preventative health strategies
- Accurate clinical documentation and record management.
- Working within Best Practice and ZOHO systems (highly regarded).
About You
You are a nurse who genuinely cares.
You enjoy building rapport with patients, solving problems, coordinating care, and working collaboratively with healthcare professionals to achieve the best outcomes.
You'll bring:
- Current AHPRA registration as a Registered Nurse.
- A robust interest in Veteran healthcare.
- Excellent clinical assessment and care-planning skills.
- A proactive and preventative approach to healthcare.
- Strong organisational and communication abilities.
- Confidence working independently while contributing to a team environment.
- A commitment to delivering exceptional patient experiences.
Experience in General Practice, Primary Care, Chronic Disease Management, DVA programs, Community Health, or Veteran Health is highly regarded but not essential.
