Join a dedicated and multidisciplinary Child & Family Health team committed to improving health outcomes for children and families across the community. As a Clinical Nurse Specialist Grade 2, you will provide advanced, evidence-based nursing care, health assessment, early intervention, and family-centred support to children from birth to five years and their families. Working autonomously and collaboratively, you will deliver comprehensive child and family health services through clinic appointments, home visits, and community outreach.
You will play a key leadership role by providing clinical expertise, supporting quality improvement initiatives, mentoring colleagues, and contributing to the delivery of secure, high-quality, culturally responsive care. This is an excellent opportunity for an experienced Child & Family Health Nurse who is passionate about early childhood development, health promotion, and partnering with families to achieve the best possible outcomes for children. The purpose of this role is to guide a centralised intake, triage and booking process for Child and Family Health and FCC services.
The model enables: Early identification of family need and risk Appropriate allocation to virtual, face-to-face, or FCCs Centres services
Efficient use of Child and Family Health nursing-midwifery resources Attend Special Care Nursey (SCN) meetings PLEASE NOTE: While this role utilises virtual care technology, it is not a remote or work-from-home position.
The successful applicant will be site-based, reporting to a designated location within the Central Coast local health district. Key highlights: Deliver advanced Child & Family Health nursing care across community settings.
Undertake child health and developmental assessments and provide early intervention. Support families through evidence-based education, health promotion, and care coordination. Provide clinical leadership, mentorship, and support to nursing colleagues.
Work within a collaborative multidisciplinary team focused on improving outcomes for children aged 0 to 5 years and families. Contribute to service development, quality improvement, and best practice initiatives. This role is suitable for:
Registered Nurses-midwives working in the Child and Family Health (CFH) Nursing team space as a Virtual Nurse CFH nurses-midwives Family Care Centre (FCC) nurses-midwives
The Nurses-midwives must demonstrate competence in virtual care assessment via learning package completion prior to being eligible for the role You will work collaboratively with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing. With a focus on health promotion, early intervention, and continuity of care, your contribution will make a meaningful difference to the lives of those you support.
Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA), current NSW Divers Licence and relevant postgraduate qualifications or extensive experience in this specialist area. Demonstrated understanding of and experience providing evidence-based health promotion, assessment, prevention and early intervention services to children and their families, particularly those experiencing health inequities Demonstrated ability to provide complex care and advanced clinical skills for families and children aged 0 to 5 years with ability to problem solve through the development of clinical management plans in consultation with any relevant stakeholders.
