Wesley Mission in Blacktown, Sydney's western area, seeks a Part time Specialist Aftercare Worker 0.8 FTE to support young people aged 18–25 exiting OOHC. You will manage a caseload and help access housing, health, legal services and education, delivering mentoring and group work as required.
You will work across Sydney districts, liaising with service providers, advocating for clients and ensuring adherence to the Social, Community, Home Care and Disability Services Industry Award.
