VMCH is seeking an organised Care Partner to manage a portfolio of Home Care Package clients across Melbourne's Northern and Eastern Metropolitan regions. You will assess needs, develop person-centred plans, and coordinate services to support independence at home.
Ideal candidates will hold a Diploma or higher, have experience in Home Care/Support at Home, and demonstrate excellent communication and computer skills with a willingness to travel as required.
