Monash Health invites applications for a Social Worker, Grade 4, within the Agile Psychological Medicine (APM) Clinics. You will join a multidisciplinary team of Psychologists, Social Workers and Psychiatrists to deliver timely, evidence-based mental health care.
You will conduct psychosocial assessments, develop recovery-focused treatment plans, provide therapeutic social work interventions, and use routine outcome measures to inform care decisions.
