Tamworth Aboriginal Medical Service – Aboriginal Corporation (TAMS-AC) is seeking an ITC Coordinator/Outreach Worker Liverpool Plains to join our team in Tamworth/Quirindi, NSW. This part-time role mixes care coordination, outreach, advocacy and practical support for Aboriginal clients with chronic health conditions.
You will work with clients, families, GPs, and other health services to ensure access to culturally protected, coordinated care and to strengthen self-management.
