Bundaleer Home Care, part of Apollo Care, is seeking a Clinical Care Partner to deliver person-centred clinical care for older Australians in their own homes. The role involves assessments, care planning, nursing interventions and coordinating with GPs and allied health to support clients to live safely and independently.
Ideal candidates hold current AHPRA RN registration, First Aid/CPR, a valid driver's license, and an NDIS Worker Screening Check, with at least 1 year in community nursing.
