Amplar Health is seeking a Cardiac Rehab Program Clinician to join the national Heart Health at Home program. This role is a part time 0.6 FTE, remote position with a 12-month fixed-term contract, delivering evidence-based cardiac rehabilitation coaching and care planning by phone or video.
You will manage your own caseload, conduct assessments, develop personalised plans, coordinate with GPs and specialists, and support patients in using the Cardihab app to improve heart health from their home
