About the role
The role is positioned within both the Community Mental Health Rehabilitation Service and the PEIPOD Service which sits under the Community Mental Health Service.
The Community Mental Health Rehabilitation team provide a trauma Informed, recovery orientated service to adults ***** with a diagnosed mental illness.
PEIPOD is a multidisciplinary early intervention service for young people ***** years of age with a recent onset of psychosis or a mood disorder.
Both services aim to provide mental health, medical and psychosocial support based on individuals' needs by specialised, goal focused support for people to move forward with their life and their recovery.
Provide assessment and psychological therapy, work collaboratively with young people across both the inpatient end community settings, and work in close partnership with local non-government organisations.
Support with the metabolic management of consumers living with mental illness through the Community Mental Health Metabolic Clinic working alongside the Endocrinologist, Dietitian, Diabetes Educator and Exercise Physiologist.
Deliver protected and effective physical health assessments, interventions and treatment for those with cardiometabolic and associated risk factors who are identified by the Community Mental Health Service.
Hold a caseload of clients that have been referred to the Rehabilitation Team and the PEIPOD team and work in a case management capacity.
This role operates Monday - Friday 8:30am - 5pm based in The O'Brien Centre, St Vincent's Hospital -Darlinghurst Campus.
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
Unrestricted NSW Driver's Licence and willingness to drive to community appointments on shift.
Demonstrated knowledge and direct practice experience in the assessment and case management of people with mental illness, including risk assessment and utilising recovery-oriented interventions.
Demonstrated effective communication and interpersonal skills as well as effective time management skills and ability to prioritise competing workloads within a complex mental health service.
Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis while working with clients experiencing complex needs including mental illness, homelessness and drug and alcohol use.
Demonstrated ability to carry out comprehensive physical health (including metabolic measures) and mental health assessment and carry out a care plan working within the multidisciplinary team.
Demonstrated in-depth knowledge and experience of the Mental Health Act **** NSW and understanding of the professional,
legal and ethical responsibilities of a Registered Nurse.
