Mind Australia is seeking a compassionate Community Mental Health Practitioner to support young people (12-25) across Mackay. This permanent full time role provides outreach-based support, with flexibility to work from office or home as needed, to help clients achieve recovery goals and meaningful community participation.
You will work with families and clinical teams, delivering individual and group supports, coordinating care and making referrals to education, health and housing services.
