Clinical Aged Care Assessor - NSW 4 Hunter
Location - travel around Hunter Region ( Newcastle, Lake Macquarie through to Port Stephens)
Permanent part time role - 3 days, adaptable with days and times
Pay rate – $***** per hour + superannuation
7 additional leave days per year (pro-rata)
Pay less tax with Not For Profit Salary Packaging
Travel will be reimbursed at 0.91 cents per km in line with our travel policy

We have a great opportunity for a Clinical Aged Care Assessor to join our Aged Care Assessment Services('ACAS') team in the Hunter Region.
ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region.
This role requires a clinical qualification/background – Physiotherapist, Occupational Therapist or Registered Nurse with AHPRA or Social Worker with AASW membership.
What will your role look like?
You will be conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care ACT
Day to day this includes:
Collaborating with clients, families and multidisciplinary teams asrequired
Working collaboratively with colleagues to promote a supportive and positive team culture
Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services
Reviewing support plans to meet client's changing support needs




Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes
Maintaining currency of knowledge relating to aged care services both funded and non-funded
Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes
To be successful in this role you'll need:
Tertiary Qualification – in a health-related disciplinedirectly relatedto Health, AgedCareor related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)
Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
An Australian Driver's license and access to a registered, insured vehicle
To be bilingual - Cantonese or Mandarin ideally (preferred but not essential)
Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia(desirable)
Willingness to apply for a Police Check
