You will support Allied Health professionals by delivering client care, helping with care plans and equipment, and coordinating referrals during home, clinic, and community visits.
You will work in a multidisciplinary team, demonstrating client-centred care for people with long-term conditions, aging needs and disabilities.
A current driver's licence and a vehicle are essential.
