Physiotherapist - Aged Care Assessor - Hybrid - Hobart, TASWe are seeking an experienced Physiotherapist to deliver comprehensive aged care assessments in Hobart, TAS.
This is a hybrid role combining half telehealth and half face-to-face assessments, offering variety and flexibility.
You will conduct comprehensive assessments to determine the care and support needs of older Australians.
Working as part of a multidisciplinary team, you will undertake holistic assessments considering clients' health, functional capacity, social circumstances and overall wellbeing, helping them access appropriate aged care services and supports.Conduct comprehensive aged care assessments across both telehealth and face-to-face settings, in accordance with relevant assessment frameworks and guidelinesAssess clients' clinical, functional, cognitive,



psychosocial and environmental needsDevelop recommendations and support plans to facilitate access to appropriate aged care servicesComplete detailed assessment documentation and reportsWork collaboratively with multidisciplinary clinicians and stakeholders$85,000 - $105,000 per annum + superannuationFull-time or part time opportunities available (minimum 4 days per week)Flexibility and variety across your working weekSupportive and team-oriented team environmentMeaningful work supporting older AustraliansCareer progression opportunities within a growing sectorAbout YouRelevant degree in PhysiotherapyCurrent AHPRA registrationStrong communication and clinical assessment skillsExperience in aged care, community health or rehabilitation highly regardedAbility to work autonomously across both remote and community settingsExcellent documentation and stakeholder communication skillsCurrent driver's licence and own vehicleReliable internet connection and suitable home working setupEligible to work in Australia
