Job Description
Clinical Care Partner – Community Services – Gorges Hall (In office)
Salary: $85,000 – $110,000 + super
Salary Packaging: Up to $15,900 tax-free + additional $2,650 per year (meals & entertainment)
Key Responsibilities
Conduct comprehensive client assessments and develop care plans
Coordinate in-home services and liaise with external providers
Provide education, guidance, and advocacy for clients and families
Monitor care budgets and ensure compliance with aged care standards
Build strong, trusted relationships with clients and stakeholders
About You
Current AHPRA Registered Nurse registration
Experience in aged care, community nursing,



or care management
Sound knowledge of the Support at Home Program and aged care standards
Strong communication, organisation, and time-management skills
Requirements
Current NSW Driver's Licence with a reliable, insured vehicle
Right to work in Australia
Valid AHPRA registration
Pre-employment medical, police check, and required vaccinations
