Physiotherapist - Aged Care Assessor - Hybrid - Hobart, TAS

We are seeking an experienced Physiotherapist to deliver comprehensive aged care assessments in Hobart, TAS. This is a hybrid role combining half telehealth and half face-to-face assessments, offering variety and flexibility. You will conduct comprehensive assessments to determine the care and support needs of older Australians. Working as part of a multidisciplinary team, you will undertake holistic assessments considering clients' health, functional capacity, social circumstances and overall wellbeing, helping them access appropriate aged care services and supports.

- Conduct comprehensive aged care assessments across both telehealth and face-to-face settings, in accordance with relevant assessment frameworks and guidelines
- Assess clients' clinical, functional, cognitive, psychosocial and environmental needs
- Develop recommendations and support plans to facilitate access to appropriate aged care services
- Complete detailed assessment documentation and reports




- Work collaboratively with multidisciplinary clinicians and stakeholders

- $85,000 - $105,000 per annum + superannuation
- Full-time or part-time opportunities available (minimum 4 days per week)

About You

- Relevant degree in Physiotherapy
- Current AHPRA registration
- Strong communication and clinical assessment skills
- Experience in aged care, community health or rehabilitation highly regarded
- Ability to work autonomously across both remote and community settings
- Excellent documentation and stakeholder communication skills
- Current driver's licence and own vehicle
- Reliable internet connection and suitable home working setup
- Eligible to work in Australia
