Home Care Packages Partner- 4 - 6 week temporary role
Care at Home Partner needed to join a dedicated Aged Care team focusing on assisting the elderly and individuals who need extra support to live independently and safely at home.
About the Role
As a Home Care Coordinator, you'll play a pivotal role in planning, coordinating, and monitoring care and support services for clients. Working closely with clients, families, and support staff, you'll ensure services are delivered with compassion, professionalism and efficiency.
To be successful in this role, you will need:
- MinimumCertificate III in Aged Care, Individual Support or ENs with home care package experience
- Current First Aid Certificate
- Valid Australian Driver's License
- Current Police Check
- NDIS Worker Screening Check
