Colac Area Health is seeking a Social Support Facilitator to join our Allied Health team in Colac. This permanent part time role (18 hours per week) focuses on delivering one-to-one and group programs for carer respite, social connections and community engagement.
You will support wellbeing through exercise, outings, education sessions, and documentation of progress using MS Word and Excel. A Cert III in Individual Support and First Aid Level 2 are preferred, plus a current driver’s licence.
