Arche Health, a not-for-profit community-based organisation in Perth, is seeking a Care Coordinator to join the Aboriginal Health Team based in Bentley. You will support clients through the ITC program, coordinating GP and allied health appointments and promoting culturally protected care.
Work with multidisciplinary teams and stakeholders to improve access to health services for Aboriginal and Torres Strait Islander people, ensuring timely care and strong community engagement.
