The Paediatric HITH Liaison Nurse will identify and coordinate suitable referrals, undertake patient assessments, provide clinical advice and telehealth support, coordinate care and ensure safe, seamless transitions into the HITH service.
The Paediatric HITH Nurse will deliver inpatient‑equivalent care in the home through home visits and community outreach, including comprehensive clinical assessment, ongoing monitoring, administration of treatments such as intravenous therapies, patient and family education, and support for safe high‑quality care at home.
May also work intermittently within the inpatient paediatric ward to support operational requirements, maintain clinical capability and strengthen partnerships.
