Mental Health Drug & Alcohol, Dubbo About the role An exciting opportunity exists for a Specialist Dementia Clinician to join Western NSW Mental Health and Drug & Alcohol Services.

Working in close partnership with the Specialist Dementia Care Program (SDCP) and other key services, this advanced clinical role provides expert leadership and consultancy to support older people living with complex dementia and mental health needs. The position plays a pivotal role in delivering person‑centred, evidence‑based care while promoting positive consumer outcomes and reducing avoidable hospital presentations. With a strong focus on in‑reach care coordination, you will facilitate integrated multidisciplinary care, support shared care planning, coordinate safe transitions between services and strengthen clinical pathways to ensure consumers receive care in the least restrictive and most appropriate environment.

Working collaboratively across community, specialist and residential aged care settings, you will apply advanced assessment, intervention and clinical decision‑making skills to support consumers, families and carers. You will also provide specialist advice and guidance to clinicians and service partners, contributing to high‑quality, coordinated care for some of the region's most vulnerable older people.
