Care Partner – Enrolled Nurse - Community Aged care

Are you an experienced, passionate Care Partner ready to make a genuine impact in aged care? or A New Graduate excited to embark on the journey to become a Care partner?

Join Senior Helpers Northern Beaches and Suburbs, a well-established and growing Support at Home Provider, and be part of a dedicated team focused on delivering high-quality, client-centred support to older Australians.

This is a Part time role based at our Dee Why Office.

Your day to day will include...

Conduct holistic client assessments and develop care plans that support their needs, maximise client choice, well-being and independence.

Conduct regular monitoring of services and care plan reviews to ensure optimal care and services are maintained to meet changing needs

Co-ordinate services and organise supports based on government compliant care plan for the client

Communicate effectively and build trust with clients and their family or representatives.

Responsible for management of your own portfolio of clients for their care, SAH budgets and all their relevant documentation. Portfolio will consist of reasonable number of clients.

Liaise and work closely with internal team members like EN, RN, scheduling and admin.

Participate in marketing and promotional events.

About you...

Proven background in managing clients within Support at Home or Home Care Package programs





Strong communication abilities—you build trust quickly and work confidently with people from varied backgrounds

Highly organised, with excellent time management and strong attention to detail

Understanding of the Support at Home framework or training can be provided

Comfortable using technology, including digital care management platforms

New graduates are encouraged to apply

Licence/Certification:

Diploma in Nursing OR Certificate IV in Ageing Support OR Diploma in Community services or equivalent

National Police Certificate (Required)

First Aid and CPR certificates (Required)

Unrestricted NSW Driver License (Required)
