NSW Health in Northern Sydney Local Health District is seeking an experienced Clinical Nurse Consultant Grade 2 to lead a Heart Failure Supportive Care project. This nurse-led role will coordinate care, guide service development and support patients and families across the district.
The successful candidate will provide clinical leadership, work with a multidisciplinary team and drive improvements in quality of life and access to supportive care services, while fulfilling NSW Health requirements.
