We have a excellent opportunity to join our Aged Care Assessment Services ('ACAS') team. ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region. We are currently looking for a Clinical Aged Care Assessor to join the team covering the Illawarra area with some travel further south.

Responsibilities

- Conduct assessments to identify and meet the complex needs of clients and refer them to appropriate aged care services under the Aged Care ACT.
- Collaborate with clients, families and multidisciplinary teams as required.
- Work collaboratively with colleagues to promote a supportive and positive team culture.
- Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
- Review support plans to meet clients’ changing support needs.
- Work effectively both autonomously and as a member of a broader team in delivering key performance indicators within identified timeframes.
- Maintain currency of knowledge relating to aged care services, both funded and non‑funded.
- Assist with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.

Qualifications

- Tertiary qualification in a health‑related discipline directly related to Health, Aged Care or a related specialist area (e.g., Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker).
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
- Australian Driver’s licence and access to a registered,



insured vehicle.
- Minimum of one year demonstrated experience in Australia or overseas directly delivering services in aged care settings and/or to aged persons including those living with dementia.
- Willingness to apply for a Police Check.
