Fairfield Hospital Pharmacy is a supportive team dedicated to delivering comprehensive clinical and operational pharmacy services that promote the safe and effective use of medicines. Working in partnership with medical, nursing, and allied health teams, we strive to optimise medication management and improve patient outcomes. Selection Criteria

To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

- Full Pharmacist registration with AHPRA with no restrictions to the scope of practice.

- Tell us about a time when you identified a medication-related problem that had the potential to impact patient safety or treatment outcomes. How did you assess the situation, what actions did you take, and what was the outcome?

- Describe a time you received feedback about your performance, knowledge, or professional behaviour. Explain how you responded to the feedback and the impact it had on your subsequent practice or approach.

- Pharmacy services involve balancing many different clinical and operational priorities. What do you consider to be one of the most important clinical activities undertaken by hospital pharmacists and why? Please provide an example from your experience to support your answer.

- Describe a situation where you needed to have a challenging discussion with a patient, carer, colleague, or other healthcare professional. How did you approach the conversation, what strategies did you use, and what did you learn from the experience?
