What you will do:




Undertake comprehensive mental health assessments and develop person-centred care plans in line with Care Programme Approach (CPA) and recovery models
Manage a defined caseload as a Care Coordinator, overseeing care delivery, reviews, and discharge planning
Deliver evidence-based interventions, including risk management, crisis response, and therapeutic support in community settings
Work collaboratively within a multidisciplinary team including psychiatrists, social workers, and occupational therapists
Qualifications:
Registered Mental Health Nurse (RMN) qualification
Current NMC registration
Registration & Compliance:
Adherence to the NMC Code of Professional Conduct
Knowledge of the Mental Health Act, Mental Capacity Act, and safeguarding frameworks
Understanding of NHS governance, CQC standards, and clinical audit requirements
