Better Health Network in Victoria is seeking a Community Health Nurse to join our Homecare Services team. You will provide person-centred nursing care in clients' homes and community settings, helping people maintain independence, wellbeing and quality of life.
You will work with adults with diverse needs, coordinating clinical care, education and services across Support at Home, Post Acute Care and other community programs.
