Use professional judgement to plan, develop, deliver and evaluate clinical social work services related to the services priority areas.

About the role:

Reporting to the Ambulatory Allied Health Service Manager, this role is integral to delivering culturally appropriate, equitable, comprehensive, and high-quality integrated care focused social work services for patients in the Ambulatory Allied Health Service. You will support patients with significant psychosocial stressors, difficulty accessing or navigating community supports and services, at risk of financial/psychological/physical neglect or abuse/compromised personal safety and/or Carer stress that, without intervention, could lead to hospital admission.

The role is based at GP Plus sites.

Duties for the role will include:

- Providing high level evidenced-based psycho-social work assessment, diagnosis and appropriate interventions for patients with complex and chronic health issues, using well-developed therapeutic social work skills.
- Working with the multi-disciplinary team, patients and their families to provide care coordination that achieves timely, safe and effective outcomes for patients that facilitate early discharge, support care in the community and prevent unnecessary hospital presentations and admissions.
- Providing expert opinion, consultancy and education services to Ambulatory Allied Health staff supporting patients with complex social work issues to maximise self-management strategies.
- Significantly contributing to the strategic planning process for social work services across relevant work areas, incorporating regular evaluation of services and reporting on quality improvement activities.
- Fostering cooperation, collaboration and coordination of service delivery with other disciplines, agencies and external service providers by participating in working parties, service reviews and representing Allied Health at a local, regional and state level as required.
- Contributing to the development of an integrated team approach that promotes realisation of the organisation's vision and mission.

About you:

- Hold a valid Australian Drivers Licence.
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Significant post graduate experience in providing clinical management for complex and vulnerable patient populations.
- Experience in working with community service referral pathways relevant to a Community/Ambulatory setting.
- Apply a high level of competence in a broad range of psycho-social work assessment, treatment methodologies and discharge planning.
- Demonstrated well developed interpersonal skills that foster trust, respect, confidence and cooperation with colleagues, patients, families, multi-disciplinary team members and, senior staff.
- Utilise high level communication skills to effectively work with, and resolve issues and conflict with, people with diverse value systems, cultural differences and special needs, including clients, multi-disciplinary health professionals,



other organisations and providers, and the community.
- Utilise effective problem solving, time management and apply specialist skills to complex issues relating to Social Work services for clients.
