Continuing Care Team provide services in conjunction with non-government and government agencies to support consumer recovery. These services provide daily follow up for consumers living in the community to support them to remain within their home setting. It is the aim of this service to offer a recovery-oriented approach that is person-centred, strengths based, collaborative and empowering.
