As a Social Worker with Veteran Pathways (part of the Proactive Health Group), you are the person who holds a client's care together from day one. About the role
As a Clinical Social Worker and Care Coordinator, you are the anchor of our Chronic Condition Management Program. You will own a caseload of veteran and Medicare clients and deliver everything by telehealth from a private, confidential setting.
