About The Team
The Caulfield Hospital team specialises in community‑based care, rehabilitation, and aged care services. Services extend across hospital, community and home‑based settings, allowing care to be tailored to individual patient needs. Pharmacists are essential members of all clinical teams, actively engaging in a unit‑ and team‑based care model involving ward rounds and collaborating closely with the multidisciplinary team to achieve excellent patient care outcomes.

About The Role
This role provides an experienced clinical pharmacist with an opportunity to lead the Health of Older People (HOP) Program, delivering care across inpatient, home‑based and community settings.
Responsibilities include leading the development and delivery of pharmacy services within HOP inpatient settings, contributing to service improvement and evaluation initiatives, mentoring junior staff and students, and advancing safe and effective use of medicines in the community.

Qualifications & Experience Required
- Registered pharmacist in Australia
- Relevant postgraduate qualification in clinical pharmacy or leadership and management highly desirable
- Extensive hospital pharmacy experience with a demonstrated understanding of contemporary practice highly desirable
- Experience in leading and coaching a highly effective team (formally or informally) desirable
