Southern Cross Care Queensland in Sydney, NSW is seeking a Clinical Pharmacist to deliver onsite medication reviews and participate in a multidisciplinary clinical setting with nurses, physiotherapists, and dieticians.
You will contribute to family case conferences, provide staff education, and engage in governance activities such as audits and policy development. AHPRA-registered pharmacists with MMR/ACOP credentials are preferred and NDIS clearance is required.
