About The Role

The Paediatric Hospital in the Home and Daystay nurses are responsible for the delivery of high quality care to patients admitted to the Barwon Health Paedaitric Homecare (HITH/PAC) and Daystay. The roles will work across early and late shifts including weekends.

As an experienced Paediatric nurse you will provide evidence based care and education to patients and families in the home environment and Ambulatory setting.

Key Responsibilities





- Provide nursing assessments and clinical care to HITH/PAC and daystay patients as per care plan
- Liaise with medical team/HITH coordinator/Ambulatory Manager with clinical concerns
- Escalate care in the community to ensure safety of patients
- Undertake home visiting risk assessments
- Liaise with HITH coordinator to discuss changes in care plan
- Assist HITH coordinator with intake and coordination
- Actively communicate with internal and external stakeholders

What We’re Looking For

- Current registration as a registered nurse with Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (APHRA)
- Experience in acute paediatric or paediatric community care
- Current Victorian drivers licence
- Extensive clinical skills and knowledge, and able to work autonomously
- Sound clinical assessment and judgement
