Key Responsibilities:

- Evaluate patients and develop individualized treatment plans in a home health setting
- Deliver hands-on therapy to improve mobility, strength, and overall function
- Educate patients and caregivers on exercises and care plans
- Track progress and adjust treatments as needed
- Complete accurate and timely documentation

Qualifications:

- Active Physical Therapist license in New Mexico
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong communication and organizational skills
- Home health experience preferred
