What you will do

- Provide specialised rehabilitation interventions for patients following a stroke, coordinating with internal and external services, including Social Services and voluntary organisations, to meet patient and carer needs.




- Conduct comprehensive physiotherapy assessments and develop short- and long-term rehabilitation plans tailored to individual patient requirements.
- Manage a caseload of patients with complex neurological needs using evidence-based, patient-centred approaches to assess, plan, implement, and evaluate treatment.
- Work autonomously under the guidance of a Band 7 supervisor while contributing as an integral member of the Neurology team.
- Deliver specialised physiotherapy services across rehabilitation centres, patients’ homes, community facilities, and hospitals to help patients achieve their rehabilitation goals.

Requirements

- Experience working with individuals following a stroke within neurological rehabilitation.
- Community physiotherapy experience is essential.
- HCPC registration is required.
- A valid driving licence and access to a vehicle are essential for community visits.
