Catholic Healthcare Limited in Newcastle, NSW is seeking a Clinical Aged Care Assessor to support ACAS in the Hunter Region. The role involves local travel and assessing complex needs to connect clients with suitable home care services.
You will conduct assessments using the IAT and collaborate with clients, families and multidisciplinary teams. A tertiary health qualification and current AHPRA or AASW registration are required.
