Do you want to support the delivery of high-quality, evidence-based stroke care across SWSLHD, contributing to improved patient outcomes and consistent standards of practice district-wide?
This role will give the successful candidate the opportunity to collaborate with committed multidisciplinary stroke teams and healthcare providers across SWSLHD, working closely with clinicians, managers and service partners to develop and implement initiatives that strengthen stroke pathways across acute, subacute and community services.
The position will have a patient centered approached, helping to enhance the experience of people living with stroke and their families through education, coordination and innovation.
Lastly, the role will play a key role in shaping district-wide stroke strategy, contributing to workforce development, capability building, and service improvement across multiple sites.

The ideal candidate thrives in a collaborative district -wide role, demonstrating leadership skills, advanced clinical expertise in stroke care, excellent communication skills and partnership building with clinicians across the district.
Futhermore, the successful candidate must be highly motivated and committed to person-centered & culturally responsive care, as well as having confidence in working across multiple sites with the flexibility to travel in between.
