**Position**: Care Coordinator

**Location**:Stones Corner Office, Brisbane

**Responsibilities**:

- Build solid relationships with clients and care workers, ensuring seamless communication and exceptional service.
- Manage a caseload of up to 35 clients living with various disabilities, coordinating their care plans, budget management, and promoting independence.
- Conduct home visits to introduce clients to our compassionate care workers and provide ongoing support.
- Utilise computer skills for efficient record-keeping and communication.

**Requirements**:

- Minimum of 12+ months of experience in the Aged and Disability Sector, preferably as a care worker.
- Valid QLD Driver's License.
- First Aid & CPR Certificate.
- Yellow Card/NDIS Worker Screening Clearance (Paid).
- Federal Police Check.
- Strong computer literacy.

Pay: $75,000.00 - $80,000.00 per year

- 8 hour shift
- Monday to Friday

Supplementary Pay:

- STONES CORNER, QLD 4120: Reliably commute or planning to relocate before starting work (preferred)
