**NDIS Support Worker - Appointment Support (Lalor VIC)**

**Location**: Unit 2/112 Mount View Rd, Lalor VIC 3075 (Back Unit)
**Date Needed**: Tuesday, 14th October
**Time**: 11:30 AM - 2:30 PM (3 hours)
**Destination**: Lalor to Flemington

We are looking for a reliable and compassionate **NDIS Support Worker** to assist a **55-year-old male participant** with **early-onset dementia** in attending his regular medical appointment.

**Duties**:

- Provide transport from Lalor to Flemington and return
- Offer companionship and support during the appointment
- Ensure participant’s safety and comfort throughout the trip

**Requirements**:
Current **NDIS Worker Screening Check**
**Driver’s licence** and **reliable car** (essential)
Experience supporting participants with **dementia or cognitive conditions is a plus**
Friendly, patient, and reliable

**Note**: Support hours may increase once everything is set and ongoing arrangements are confirmed.

**Job Types**: Part time, Casual

Pay: $32.00 - $35.00 per hour
