You will be part of the Chronic and Complex (CAC) Allied Health team within Western Sydney Local Health District, working across Auburn and Parramatta Community Health Centres. Our collaborative, community-focused team is dedicated to improving the health, mobility and wellbeing of older people. What you'll be doing
As a CAC Physiotherapist, you will play a key role in restoring, maintaining and enhancing the physical function, mobility and independence of older adults living with chronic, complex or age-related conditions. Working across community settings, including home visits and telehealth, you will provide comprehensive assessments, evidence-based interventions and client-centred care plans.
In this role, you will:
- Deliver high-quality physiotherapy assessment, intervention and education aligned to the CAC Model of Care
- Manage and prioritise intake, waiting lists and scheduling using sound clinical reasoning
- Develop and review measurable goals and care plans with clients, carers and the multidisciplinary team
- Conduct holistic assessments such as falls risk, delirium screening and pressure injury risk
- Maintain professional standards through ongoing education, supervision, and performance development

Essential Criteria:
- Qualification: Tertiary qualifications in Physiotherapy
- Registration: Current general Physiotherapy registration under AHPRA
Selection Criteria:
1. Demonstrated experience and skills in Physiotherapy assessment, planning and evaluating interventions for clients with chronic and complex health conditions.
2. Demonstrated understanding of the health and associated care needs of persons with complex, chronic and/or aged conditions residing in the community.
3. Demonstrated ability in the use of relevant evidence based, best practice guidelines to guide decision making for best client outcomes.
4. High-level written and oral communication skills, computer competence and ability to use electronic medical records,
word processing and databases.
5. Ability to work independently and as part of multidisciplinary team.
6. Current Drivers Licence valid within Australia.
