We're looking for Nurse Practitioners (NPs) and Physician Assistants (PAs) interested in a adaptable PRN role conducting in-home Health Risk Assessments (HRAs) for Medicare Advantage members.

What you'll do:

Share preventive care education—no ongoing management required.
Travel locally to patients in your county and nearby areas.
What we're looking for:
Active NP or PA license (excluding Pediatrics, PMHNP and Women’s Health specialties).
Minimum availability of five full 7.5-hour days per month.
Willingness to travel within roughly 60 minutes of your home.
Additional info:
