About the Role
/n Are you an experienced case manager who is passionate about helping older people live safely, independently and with dignity in their own homes? /n /n 3Bridges is looking for a Care Partner to join our Aged Care team on a full-time, fixed term (18 months) parental leave contract based at Carss Park. /n In this role, you’ll manage an allocated caseload, building trusted relationships with clients, families and carers while developing and reviewing individual care plans, coordinating services and responding to changing client needs. /n You’ll work collaboratively with frontline workers, the Rostering team, and other internal services to ensure clients receive responsive, person-centred support that promotes their independence, safety and wellbeing. /n /n Why Join 3Bridges? /n /n

Key Responsibilities
/n /n
- Build trusted relationships with clients, families, carers and advocates /n
- Manage an allocated portfolio of aged care clients /n
- Develop, implement and regularly review individual care plans /n
- Conduct client consultations, welfare checks and home visits /n
- Monitor changing client needs, risks and safeguarding concerns /n
- Coordinate appropriate services and supports /n
- Support clients to achieve their individual goals /n
- Manage client budgets in line with funding requirements /n
- Maintain accurate care plans, case notes and client records /n




- Work collaboratively with frontline workers, Rostering and internal teams /n
- Ensure services are delivered in accordance with relevant aged care quality and compliance requirements /n /n /n About You /n You’re an experienced and compassionate case management professional who enjoys building strong relationships with clients and their families, along with a genuine commitment to person-centred care. /n Essential Requirements /n /n
- Tertiary qualifications in a Human Services Discipline, i.e. Social Work/Psychology or similar area /n
- Demonstrated case management experience within aged care, community services or a related field /n
- Strong communication, organisation and relationship-building skills /n
- Ability to manage care plans, client records and competing priorities /n
- Current unrestricted driver’s licence /n
