Liverpool University Hospitals NHS Foundation Trust is seeking a energetic Ambulatory Palliative Care Clinical Nurse Specialist to join the IMPaCT Hub team based at Woodlands Hospice.
You will manage a caseload of community patients, run nurse-led clinics, and triage referrals in collaboration with hospital, hospice and community teams.
Applicants should have palliative care experience and role-specific training; advanced communication and Advance Care Planning skills are desirable as you will
