Wise Healthcare is seeking a reliable, compassionate, and dedicated NDIS Support Worker to provide support to a participant located in Byfield, Queensland.

The successful applicant will assist the participant in meeting their support needs while promoting independence, dignity, safety, and overall wellbeing. We are looking for someone who is patient, respectful, dependable, and committed to delivering person-centred support.

Key Responsibilities

- Provide support in accordance with the participant’s individual needs and preferences.
- Encourage independence, choice, and community participation where appropriate.
- Build a positive and professional relationship with the participant.
- Follow the participant’s support plan and all relevant workplace policies and procedures.
- Maintain accurate progress notes and report any concerns or incidents to the relevant team.
- Respect the participant’s privacy, dignity, and confidentiality.
- Deliver protected, respectful, and person-centred support.

Expected hours: 2.0 per week
