Just Better Care Melbourne North and North West is hiring a Care Partner to manage a caseload of around 70 Support at Home participants, ensuring the right plan, team and funding are in place.
You will assess needs, build care plans with families and health professionals, coordinate rostering with the team, monitor plans, manage participant budgets and build robust relationships with GPs and allied providers.
