Safelane Healthcare Pty Ltd is seeking a compassionate and dedicated Support Coordinator to join our team on a casual basis . Based in Wheelers Hill, VIC 3150 , this role is fundamental to our mission of providing high-quality aged and disability support services to our clients.

As a Support Coordinator, you will play a pivotal role in connecting clients with the services and resources they need to maintain independence, dignity and quality of life. Working within the aged and disability support sector, you will be responsible for assessing client needs, developing tailored support plans and coordinating the delivery of appropriate services. This position directly impacts the wellbeing of vulnerable members of our community and is essential to the effective functioning of our support services.

Key responsibilities

- Work with NDIS participants to understand their goals, needs and support requirements.
- Assist participants to implement their NDIS plans and connect with suitable services and providers.
- Coordinate supports, monitor progress and help address barriers to achieving participant goals.
- Advocate for participants’ choices, rights and independence.
- Build and maintain effective relationships with participants, families, providers and community services.
- Maintain accurate case notes, service records and documentation in line with NDIS requirements and organisational policies.
- Identify emerging risks or changes in participant circumstances and take appropriate action.

What we're looking for

participant-focused support.

- Experience in NDIS Support Coordination, case management or disability and community services.
- A sound understanding of the NDIS framework, participant plans and available community supports.
- Strong communication, relationship-building and problem-solving skills.
- The ability to manage competing priorities, work independently and maintain accurate records.
- A commitment to participant choice and control, privacy, safeguarding and professional boundaries.
- Relevant qualifications in disability, community services, social work or a related field.
- A current driver’s licence and reliable transport are desirable. A current NDIS Worker Screening Check and any other clearances required for the role are expected, or you must be willing to obtain them where applicable.
